Shay Maz Aesthetics & Wellness (“we,” “us”) is a medical aesthetics practice owned by Shana Mazgaj, MSN, FNP-BC, located at Canal Club 62, 2nd Floor, 62 Webster St., North Tonawanda, NY 14120, serving Buffalo, NY. This policy covers this website and the ways you contact us through it.

Information we collect

• Information you give us. When you use our contact form we receive your name, email address, phone number (optional), the topic you choose and your message. When you text, call or email, we receive your contact details and whatever you choose to share.
• Technical information. Like most websites, our web host automatically records standard server logs (such as IP address, browser type, pages requested and the date and time) for security and troubleshooting. Contact-form submissions also record the IP address they came from, to help prevent abuse.
• Spam protection. Our contact form uses a hidden field and a time check to filter automated spam. It does not use third-party tracking or reCAPTCHA.

Please don’t send medical details through the contact form. Health information is discussed privately at your consultation.

Online booking, payments and financing

Appointments are booked through Jane (shaymazaesthetics.janeapp.com), and payment plans are offered through Cherry (withcherry.com). When you use those services, the information you provide is collected by them and handled under their own privacy policies, along with our patient privacy practices. Links to ZO Skin Health and Fullscript take you to those companies’ sites, which have their own policies.

Cookies and tracking

This website does not use advertising cookies, analytics cookies or tracking pixels. If you change settings in our Accessibility tools, your choices are saved in your browser’s local storage on your device only. If you turn on the tools’ Dictionary feature, the word you select is sent to the free dictionaryapi.dev service to look up its definition. Google Maps loads only if you choose “Show interactive map,” and the Cherry payment calculator loads only if you choose to show it; those services may set their own cookies when they load.

How we use your information

• To answer your questions, schedule appointments, home visits, parties and events, and provide the services you request.
• To keep this website secure and prevent spam and abuse.
• To meet legal, tax and record-keeping obligations.

We do not sell or rent your personal information, and we do not share it for cross-context behavioral advertising.

When we share information

We share information only with service providers that help us run the practice and this website, such as our web host and email provider, and only as needed for them to provide their services. We may also disclose information if required by law or to protect the rights and safety of our clients, our practice or others.

How long we keep it and how we protect it

Contact-form messages are stored in a protected file on our web host and sent to our email, and are kept only as long as needed for the purposes above or as required by law. This website is served over an encrypted (HTTPS) connection, and we use reasonable safeguards to protect your information. No method of transmission or storage is completely secure.

Your choices and rights

You can ask us what personal information we hold about you, and ask us to correct or delete it. Depending on where you live, you may have additional rights under state privacy laws, and we will honor applicable requests. We will not discriminate against you for exercising your rights.

Text messages

If you text us, standard message and data rates may apply. We use your number only to communicate with you about your inquiries and appointments. Tell us if you’d prefer we not text you.

Children

This website is not directed to children under 13, and we do not knowingly collect personal information from them online.
